CORVISTA® HEALTH PRIVACY POLICY

This Privacy Policy explains how we handle the personal information that you provide to us on the website controlled by CorVista Health, Inc. and its parent company, Analytics For Life Inc. (collectively, “CorVista”, “we”, “us”, or “our”), staging.analytics4life.com (“Website”).

This Privacy Policy describes the types of information we may collect from you or that you may provide when you visit our Website and our practices for collecting, using, maintaining, protecting, and disclosing that information. This Privacy Policy only applies to websites that link to this policy. Our Website may also contain links to third-party websites or resources, and this Privacy Policy does not apply to unaffiliated sites, even if you access them via a link on our site. You should review the privacy policies of any third-party site before providing information.

This Privacy Policy also does not apply to personal information collected from you offline.

Please read this policy carefully to understand our policies and practices regarding your information and how we will treat it. If you do not agree with our policies and practices, your choice is not to use our Website. By accessing or using our Website, you agree to this Privacy Policy.

HOW DOES THIS PRIVACY POLICY DEFINE “PERSONAL INFORMATION”?

“Personal Information” means information that identifies you or could reasonably be used to identify you and which is submitted to and/or collected by the CorVista Website and maintained by CorVista in an accessible form. Examples of such Personal Information include, but are not limited to, your name, your postal address, your email address, and your telephone number.

HOW AND WHY CORVISTA COLLECTS INFORMATION ONLINE?

CorVista collects information you submit to it directly and passively through tracking technologies.

Personal Information You Submit:
CorVista collects Personal Information that you enter into data fields on the CorVista Website. For example, you may submit your name, postal address, e-mail address, in order to access service, receive information about various subjects, contact CorVista, or respond to CorVista emails or surveys. To protect your privacy, you should not provide CorVista with any information that is not specifically requested. Please be aware that certain features of this site may not be available to you if you elect not to provide certain information.

Passive Collection of Unidentifiable Information:
As you navigate through and interact with our Website, we may use automatic data collection technologies to collect certain information about your equipment, browsing actions, and patterns, including:

  • Details of your visits to our Website, including traffic data, location data, logs, and other communication data and the resources that you access and use on the Website.
  • Information about your computer and internet connection, including your IP address, operating system, and browser type.

The information we collect automatically does not include Personal Information, but we may maintain it or associate it with personal information we collect in other ways or receive from third parties. It helps us to improve our Website and to deliver a better and more personalized service, including by enabling us to:

  • Estimate our audience size and usage patterns.
  • Store information about your preferences, allowing us to customize our Website according to your individual interests.
  • Speed up your searches.
  • Recognize you when you return to our Website.
  • Improve service.
  • Compile statistics, analyze trends, and otherwise administer and improve the CorVista Website.

The technologies we use for this automatic data collection may include:

  • Cookies (or browser cookies). A cookie is a small file placed on the hard drive of your computer. You may refuse to accept browser cookies by activating the appropriate setting on your browser. However, if you select this setting you may be unable to access certain parts of our Website. Unless you have adjusted your browser setting so that it will refuse cookies, our system will issue cookies when you direct your browser to our Website.
  • Flash Cookies. Certain features of our Website may use local stored objects (or Flash cookies) to collect and store information about your preferences and navigation to, from, and on our Website. Flash cookies are not managed by the same browser settings as are used for browser cookies.
  • Web Beacons. Pages of our Website may contain small electronic files known as web beacons (also referred to as clear gifs, pixel tags, and single-pixel gifs) that permit us, for example, to count users who have visited those pages or opened an email and for other related website statistics (for example, recording the popularity of certain website content and verifying system and server integrity).
  • Google Services. CorVista uses Google© Analytics first-party “cookies” and potentially other partner third-party “cookies” for reporting and advertising. These first- and third-party cookies are used together to help us better understand your interests and online behavior in an aggregate and anonymous form. The Google Analytics first-party “cookies” are used to compile statistical information and reports on aggregate visitor activity on our Website. We use third-party cookies to enable Google Analytics demographics and interest reporting, which allows us to understand, in aggregate, the age, gender, and interests of our visitors. Certain features of the CorVista Website may not work without use of passive information collection technologies. Information collected by these technologies cannot be used to identity you without additional identifiable information.

CorVista will use your Personal Information on our Website for many reasons, including:

  • To present our Website and their contents to you;
  • To respond to your questions or requests and to provide you with efficient customer service;
  • To personalize your experience. For example, after you have entered Personal Information into a form or data field on the CorVista Website, CorVista may use certain identifying technologies to allow that Website to “remember” your personal preferences, such as sections of that Website that you visit frequently and, if you choose, your user ID;
  • We may also use this Personal Information for other business purposes, such as to offer you the opportunity to receive notices regarding CorVista products or services, to invite you to participate in surveys about our products, or to notify you about special promotions;
  • To complete a transaction;
  • To perform website analytics and measure website performance;
  • To maintain the Website, including for security purposes.

Some of our applications and mobile-optimized sites may have location-based features. To deliver these features, we will collect and use location data provided by your mobile device. Location information will be collected solely to deliver the requested feature, and it will not be further retained or stored. By using such applications, you consent to the collection and use of your mobile device location data for purposes of delivering these location-based features.

DOES CORVISTA CONSOLIDATE PERSONAL INFORMATION?

CorVista may combine Personal Information that you provide to us via the CorVista Website with Personal Information provided by you via other means, whether online or offline, as well with your Personal Information that may be contained in records provided by third parties. We use this consolidated information to help us better design the CorVista Website and products, to communicate information to you, to enhance our marketing and research activities, and to facilitate other business functions.

WHAT CHOICES DO I HAVE ABOUT HOW CORVISTA COLLECTS AND USES PERSONAL INFORMATION ABOUT ME?

You may always limit the amount and type of Personal Information that CorVista receives about you by choosing not to enter any Personal Information into forms or data fields on the CorVista Website. Some of our online services can only be provided to you if you provide us with appropriate Personal Information. Other parts of the CorVista Website may ask whether you wish to opt out or opt into our contact lists.

The CorVista Website may use tracking technologies to collect unidentifiable information and does not generally provide you with the ability to opt out of these tracking technologies. Some Internet browsers allow you to limit or disable the use of tracking technologies that collect unidentifiable information. If you disable or refuse cookies, please note that some parts of this site may then be inaccessible or not function properly.

DO NOT TRACK DISCLOSURE

Certain web browsers may allow you to enable a “do not track” (“DNT”) option that sends signals to websites you visit indicating that you do not want your online activities tracked. This is different from blocking cookies as browsers with the “do not track” option selected may still accept cookies. Right now, there is no industry standard for how companies should respond to “do not track” signals, although one may be adopted in the future. We do not respond to “do not track” signals at this time. If we do so in the future, we will modify this Privacy Notice accordingly. More information about “do not track” is available at www.allaboutdnt.com.

WHO WILL HAVE ACCESS TO PERSONAL INFORMATION ABOUT ME?

Personal Information can be accessed by a restricted number of CorVista employees and by individuals and entities with whom CorVista contracts to carry out business activities for CorVista. We train our employees about the importance of privacy (including without limitation as it relates to the Health Insurance Portability and Accountability Act of 1996, or “HIPAA”) and how to handle and manage your Personal Information appropriately and securely. In addition, CorVista’s practice is to require its contractors to keep your Personal Information confidential and to use Personal Information only to perform functions for CorVista.

DOES CORVISTA SHARE PERSONAL INFORMATION WITH THIRD PARTIES?

The privacy and security of your information is important to us. As a general rule, CorVista will not sell, lease, share, rent or barter any of your Personal Information to or with third parties, except as set forth in this Privacy Policy, (including if it is necessary to conduct a joint program or to carry out a program or activity through a contractor). Where required by law, CorVista will not disclose your Personal Information to third parties for those third parties’ marketing purposes unless you have been provided with an opportunity to opt into or opt out of this disclosure. However, if CorVista merges with or is acquired by another company, for example, or if CorVista sells one of its product lines or divisions, your Personal Information may be transferred to the buyer so that the buyer can continue to provide you with information and services. If CorVista provides your Personal Information to individuals and entities that assist us with our business activities, it is CorVista’s practice to require those individuals and entities to keep your Personal Information confidential and to use your Personal Information only to perform functions for CorVista. CorVista reserves the right to disclose your Personal Information to respond to authorized information requests from government authorities, to address national security situations, court orders, or when otherwise required by law.

The CorVista Website may contain links to other websites (“third-party Websites”). This Privacy Policy does not apply to third-party Websites. CorVista is not responsible for, nor do we endorse, the privacy practices of these sites.

HOW DOES CORVISTA PROTECT PERSONAL INFORMATION?

To protect your Personal Information from unauthorized access and use, we maintain a layered approach to physical and logical security controls. These measures include computer safeguards, access control and various other controls. Even so, it is impossible for CorVista to give a 100% guarantee on the security of Personal Information you provide. CorVista makes no warranties or representations as to the security and confidentiality of any data you transmit. Any data you transmit to CorVista, including your Personal Information, is done at your own risk. For instance, any information that you transmit to us over the Internet, including via email, may be unsecured and unencrypted before it reaches our servers, and you understand that third parties may be able to obtain and use that information.

CorVista does not knowingly collect or use any Personal Information from children (we define “children” as minors younger than 13) on the CorVista Website without the permission from that child’s parent or guardian. If you are a parent and become aware that your child has provided us with information, please contact us using one of the methods specified below, and we will work with you to address this issue.

If you are under 13, do not use or provide any of your Personal Information, such as, for example, your name, your postal address, your email address, or your telephone number.
